If you were a former member in good standings with the department and the association, please complete the form below. We do require the past member must have completed a minimum of 10 years of service with the association and the former member was not terminated from their employment by the city. Any exceptions to this will be considered on a case by case basis by the association board members. The board always reserves the right to refuse an applicant without specifying any reason.
Once we receive your information and confirm your status, you will sent an email with your username and password so you can view the "Members Only Section". This process should not exceed 48 hours under normal circumstances..
Once you are logged into the site we welcome you. The current members will never forget those who came before us. If you know of a past member that has reached the end of their watch, please let us know. If you know of a past member who is having a great deal of difficulties, either financially or medically, please let us know. We may be able to help.

Each line of the form allows for 20 spaces with the exception of the phone and it allows for the 10 digit number and the comment line is limited to 4 lines, each with 20 spaces in length.
